I have managed to reproduce that exact impression using ALSA, by muting
the PCM volume while setting the master volume loud, when there is
playback.

So this might turn out to be an issue with the control of the PCM
volume, in a similar way to what Julien suggested. I have not tried to
fiddle with the modules, though; in all cases this is something that
should work out of the box.
