Package: pulseaudio
Version: 4.0-6+b1
Followup-For: Bug #541538
I'm experiencing the same problem. Found a solution here:
https://wiki.archlinux.org/index.php/PulseAudio_%28%D0%A0%D1%83%D1%81%D1%81%D0%BA%D0%B8%D0%B9%29#Volume_gets_louder_every_time_a_new_application_is_started.
Uncommenting and changing
"flat-volumes = yes" to "flat-volumes = no"
in /etc/pulse/daemon.conf
solve the problem for me.
