Yes, it still happens with version 4.

$ pulseaudio --version
pulseaudio 4.0

It's also possible that some of the media files might have their own
issues but I am unable to know if that is the case or not.

sure, while I did reproduce the issue with 4, will wait for 5 to come
and see if that changes anything.
