Does the problem still happen with pulseaudio 5.0? If so, does this
happen again after `killall pulseaudio`?
Please try running pulseaudio in verbose mode:
pulseaudio -k ; pulseaudio -vvvv
And attaching the output of pulseaudio when the problem occurs.
