Bug#739100: pulseaudio: Default to using all outputs

People usually have multiple sound outputs these days (e.g., both a sound
card and HDMI); complaints that no sound is playing out-of-the-box are
frequent.

If PA would default to just plaing on all local outputs, this problem would
be solved; if that is not what the user wants, temporary hardware
workarounds (i.e. turn down the "hardware" volume knob on one of the
outputs) are easier than the other way round.
