I am experiencing the same problem. Sometimes, disconnecting the headphones doesn't route sound via the speakers. If I do connect my headphones, I will often have to open pavucontrol and select 'Speakers' instead of 'Analog Output'. The reverse sometimes happens when I connect my headphones.
The only change I have made to my default.pa is adding 'tsched=0' after 'load-module module-udev-detect'.
