You should not install that file in /usr/share/doc/

No, but you should copy its content in debian/copyright.

Sure. The best way to do that is to use reportbug, but you can do that
manually too.

Regarding sponsoring, here are some random comments on the package (only
looking at the .diff.gz):

Priority -> extra

Standards-Version: there's a newer version available

The description is a bit suboptimal:
- usually, think of the short description as something that fits
  <packagename> is a <short description>. Also, we usually try to avoid
judging the package using adjectives like 'Classy' in the description.
- in the long desc, start by describing what the package is, not what it
  isn't.
- you can probably drop the whole second paragraph.

Since you are using a patch system, it might be a good idea to turn your
change of lib/sinatra/main.rb into a patch, and also add a description
to explain why that patch is necessary.
