This is quite old but I have no idea if the problem persist.
Sorry you did not have any reply at the time.

I do not use fcgi, so I never encountered such error. I did not find any 
similar issue in the Redmine tracker either.
If you're still having this problem and can reproduce it with 4.0.4, 
please tell us and have a look at the server.errorlog file to see if 
there's more info please.
