It looks like it thinks that there is a TDBsam file there because the
configuration file has been set up that way, but can't find any (hence
the "TDBSAM version too old (0), trying to convert it." message).

This message has changed recently, it now says:

tdbsam_open: Converting version 0 database to version 3.

(just reproduced locally)

Since none of the policies are specified in the SAM file yet it gives
those warnings about returning 0.

Other than the fact that the output is a bit strange, I don't think
there's anything going wrong here.

Please note that TDBsam is not the same thing as a TDB file - TDB
is a generic database file format, TDBsam is a SAM (User/Group account
and policy database) implemented using TDB. When you store your LDAP
password, it happens in secrets.tdb, not the TDBsam (passdb.tdb).
