Try the same thing after this:
echo "QUILT_PATCHES=3Ddebian/patches" >> ~/.quiltrc

You can also try to set this variable in the shell directly:
QUILT_PATCHES=3Ddebian/patches quilt push 132_vi.dpatch

It's supposed to be in shadow-4.0.3/debian; again, "make -C trunk" will
handle these things for you. No idea why my previous mail didn't go through.
