which is covered by the GNU General Public License:

an appropriate copyright notice includes the name of the copyright holder.
So pleae add it. usualy you can simply cut&paste all those "(C)" or "(c)"
or "copyright" line.
"disclaimer of warranty" is also needed, usualy you need to copy this block:

it under the terms of the GNU General Public License as published by
the Free Software Foundation; either version 2 of the License, or

only the last paragraph can be replaced, the rest is required.
IT IS important to check whether the source uses or omites the
", or ... any later version." part, so please quote that.
