Good news: I don't see anything wrong with the files you sent.

Bad news: I don't understand what happen.

Do you remember if you recently change the PAM configuration?
Did you recently update a package?
Did you encounter a disk failure?

If you have debsums, can you try to run debsums libpam-modules libpam0g
(it checks wether the 
You may try to reinstall libpam-modules and libpam0g.

Some other questions:
 * did you install PAM by yourself without using the Debian packages?
 * does the problem occurs on the machine from which you sent the email (a
   Sarge machine)?
 * Do you have problems with other tools which use PAM (for example sudo,
   gdm, etc.)
