Did you checked if libpam-usb is installed?

Also could you send you PAM configuration file for su (/etc/pam.d/su), and
the files included by that file (probably /etc/pam.d/common-auth,
/etc/pam.d/common-session, /etc/pam.d/common-password, and
/etc/pam.d/common-account)

Can you check if /lib/security/pam_usb.so exists?
