Interesting read.  The only suggestion I can provide to you is that
the initrd somehow must launch two inits.  I have no idea how it can
happen, but it seem unlikely that the kernel starts to initrds, and
there is nothing I am aware outside the initrd that start init.

Good to hear that it is solved, even though you do not know how it was
solved.  I hope you can figure out exactly what went wrong, so we can
be sure it do not happen again, but do not expect that to happen,
given the amount of debugging that has gone into this so far. :)
