So you are using a standard initramfs according to this info.

The mounts look normal though.  You've got all the standard
filesystems mounted, plus your own.

Have you ever edited the initscripts under /etc/init.d or
the shell libraries under /lib/init?

Can you try installing initscripts/sysv-rc/sysvinit/sysvinit-utils
with 'dpkg --force-confnew -i' to make it update all of the scripts
to their current versions?  You'll need to download the testing
versions if they aren't in /var/cache/apt/archives.
