   * What led up to the situation?

I tried using systemd-sysv and wasn't happy with it so I installed 
sysvinit-core, however, with it installed, kexec-load is apparently not
running and I get a hardware reboot on init 6 rather than kexec running.

   * What exactly did you do (or not do) that was effective (or
     ineffective)?

I tried getting /etc/init.d/kexec-load to send extra output to the console,
and also put a wait command in /etc/init.d/reboot to see if any messages 
were being output before rebooting, but I saw nothing to indicate that 
/etc/init.d/kexec-load was run or any kexec-related activity generated output.

   * What was the outcome of this action?
   * What outcome did you expect instead?

kexec to work.
