The update in question was just going to a new version of the same ABI: 

from linux-image-2.6.23-1-686 2.6.23-1
to linux-image-2.6.23-1-686 2.6.23-2

No backup of the old kernel/initrd was made and no extra item was added
to the grub boot menu.
