Sounds more like an issue within nm-applet then network-manager.

Could you please run
/etc/init.d/network-manager stop
killall nm-system-settings
nm-system-settings --debug 
--config=/etc/NetworkManager/nm-system-settings.conf
NetworkManager --no-daemon
