Well, that sounds pretty weird.
One more possible reason I could think of would have been, that
gnome-icon-theme ships the nm-no-connection icon in different
resolutions (where nm-gnome only has a 22x22 icon).
But as you can no longer reproduce the problem (and I don't run lxde
myself), it will be hard to track down this issue.
