Can you provided a verbose debug log from NetworkManager when that
happens? https://wiki.gnome.org/Projects/NetworkManager/Debugging has
some debugging hints.
How are the passwords stored? System-wide or in the user
session/keyring? If the latter, which desktop environment is this?
When you resume, is your desktop session locked? Is your keyring locked
as well?
