Bug#568828: mkd.vim: wrong interpretation of _ inside [[/]]

While the underscore (_) is correctly interpreted in URL constructs
like

  [description](link_with_underscore)

and

  [description][my_link]
  [my_link]: link_with_underscore

the markdown syntax plugin doesn't properly handle

  [[description|link_with_underscore]]

or

  [[!command link_with_underscore]]

and renders the rest of the document bold-italic, which is not very
nice.
