Bug#568829: mkd.vim: line breaks confuse syntax highlighting

Markdown is happy with linebreaks in its special commands. The
mkd.vim syntax highlighter is not.

While

  [a page](link_with_underscore)

works fine,

  [a
  page](link_with_underscore)

does not — the underscore is interpreted as if I wanted the rest of
the document to be emphasised (see #568828).
