Bug#357813: spandsp: src/spandsp/mmx.h is not GPL and has no license attached

Hi,

src/spandsp/mmx.h is not under GPL, and it basically has no license
attached.

The copyright notice doesn't grant permission to redistribute the file,
nor to modify it.

Thus the file has to be removed and replaced with something like:

#undef USE_MMX
#undef USE_SSE2

(at least this works in the iaxmodem package, but it seems like the file
is unused as neither USE_MMX nor USE_SSE2 are defined)
