(This means that any external Asterisk applet needs patching in order to
build)

This change generally makes sense as it places asterisk.h in the same
relative place for external builds as it is in internal builds.

Anyway, at the time I have asked some Asterisk developers about it, and
was told it will be incorperated into trunk. I now see that it isn't. So
this needs to be followed through.
