asterisk 1.4.21 patch for rfc2833 dtmf

Hi,
i've found the rfc2833 dtmf support quite unusable (tested against
swisscom +41 0800800800) in debian lenny asterisk version, the issue
has already been resolved upstream here

and i'm attaching the same patch updated to apply cleanly in the
debian package so you just need to add a line in
debian/patches/series.
Before the patch was impossibile to use that specific ivr now it goes
fine with the first digit.
It would be nice if the patch could be applied to a 1.4 branch in svn
so i don't have to track it myself :) if it is not possible i think
the patch would be indeed useful for the other lenny users that follow
the list.
