asterisk-addons and docs: so far so good

One potential issue with asterisk-addons is that there is a single
doc/addons-en_US.xml file for all the modules. The XML docs are also the
new format in Asterisk for the run-time help. Luckily at this stage most
addons simply don't use XML docs, and still use the original simple text
interface. Thus there should be no problem so far.

In the long run (by Asterisk 1.6.3) the separate addons package should
be removed.

Anyway, asterisk-addons seems to be ready for upload.
