Rarely when I shutdown a guest, the DomU freezes and stays visible in the xm list output. 

It does not always happen so I have not been able to track it down, but xm vcpu-list showed the DomUs cpu(s) as being paused if memory serves me correctly.

The fix at the time was to simply run xm destroy, then using the same config, xm create, and all was fine.
