I don't know if it's the same/similar cause, but "... blocked for more than 
120 seconds." sounds familiar and caused me to get rid of my mdadm RAID1 
system and just use a single disk. I used LVM on top of that. I still use LVM, 
but no longer on top of software RAID.

I don't have any experience with qcow2 images though.
I'm not even sure whether my 'fix' fixed the problem (properly/entirely).

Only thing I am sure of is that your symptoms sound familiar.
