I have never seen something like this before. It really seems that
your scanner has a serious problem. I checked the log, and it looks
OK.
What you have in the 150dpi image is not a misregistration of
the color channels: You are getting the green data twice. The copy
is sent in place of real image data that is lost: When you look
at the first line, the letters "HAT" only show the bottom portion,
the top is missing.
