Try

SANE_DEBUG_AVISION=255 scanimage -L

(or whatever is needed to set environment variables on MacOS X). You
should get debug messages like this:
[sanei_debug] Setting debug level of avision to 255.
[avision] sane_init:(Version: 1.0 Build: 182)
[...]

If this doesn't happen, try:
SANE_DEBUG_DLL=255 scanimage -L

and send the output.

If you haven't tried yet, run scanimage as root.
