Firstly this isn't grave.  Everyone who doesn't use LVM can use the package so 
it's quite usable.

The core problem seems to be a semaphore created by an unconfined_t domain that 
is used by lvm_t.  Did you initially run the lvm utility when it was 
mislabeled?

Can you reproduce this after a reboot?
