I think we somehow need a "normal" modus of operation which simply
assumes unchanged mailboxes (which seems reasonable - even if there
is some chance to drop mails).

Than we need some "initial" mode of operation which fetches everything
from scratch if we have some signs that things might have changed (after
a massive SPAM effort or so).  But this is probably the exception and
could be manually triggered somehow.
