docs/HowToTheme.html covers this, but it could be more clear, and it looks
like there used to be an image to illustrate this point.

No, just the new format with a single character description per line. The
code that used the old format was broken anyway, IIRC, so I tried to simplify
it and clean it up. docs/HowToTheme.html does describe the current
keyboard.lst format pretty clearly, I think.
