This is conveniently explained in the exim specification, delivered for
exmple in /usr/share/doc/exim4-base/spec.txt.gz

Chapter 3 "How exim receives and delivers mail" is a good start.

It's a tad more complicated as it would be appropriate for a mailing
list message.
