[xml/sgml-pkgs] Bug#400314: docbook2html and docbook2pkdf do not handle upper case norwegian characters

I see a similar problem with the attached docbook fragment.  The lower
case Norwegian letters, æøå, work fine.  The upper case Norwegian
letters, ÆØÅ, fail to work.  This block my try to convert and translate
a moinmoin wiki document via docbook to PDF  and HTML. :(
