I guess you are not using dovecot auth. (#551106), are you?

Could you try whether moving away the contents of /var/spool/exim4/db/
(please do not delete, but keep a copy) fixes the issue?

What do the corrsponding entries in /var/log/exim4/mainlog* look like?
