Maybe you are using split config and need to make the change in
/etc/exim4/conf.d/main?

First step to debuging is trying to reproduce the issue with echo
"something" | /usr/sbin/exim4 target-address@example.com, and then
debugging by adding the -d switch.

Again this sounds like you need to debug.

Probably. The swaks package might help here, as it reduces complexity
on your side.
