As far as integrating spamassassin with exim, using either exim's built 
in acl or sa-exim allow you to reject spam at smtp.  The main difference 
(as far as I know) is that exim will not allow spamassassin to add 
headers, or modify the message in any way, requiring you to set that up 
yourself.  Sa-exim will let you make use of spamassassin's built-in 
header reports and message rewriting features and also includes some 
other features.

And of course, sa-exim is much easier to set up.  The main thing you 
want for proper integration with exim is to be able to reject 
high-scoring mails which both options allow, so either way is the right way.
