[Pkg-exim4-users] spec_html/*.html should have a link to TOC: URL for reportimg upstream

I've modded the generation scripts to add a TOC link to the bottom of
each chapter, and make the title of each chapter a link to the TOC.  I
can see no open bug for this, so have not linked it to the bug report.

This change will be in 4.81 when it is released, which I suspect will be
a relatively short turn round.

There is a bug report - http://bugs.exim.org/show_bug.cgi?id=1114 - for
missing items from the html docs tarballs.  This explains the other
issues discussed within this thread.
I'm currently discussing this with some of the other maintainers.
