Last steps to Mame 0.148

Hello Folks!

in the last few days I've been working on 0.148 and I think I'm nearly done.

There's a couple of things I'd like to have some feedback on before
pushing the new debian/0.148 branch:

* I removed examples for Mame and Mess .ini files (agreed on the list
not long ago).

* Newer gcc/g++/patch releases don't require stripping trailing CR and
scanning the merged source tree was increasingly prone to potential
errors as new file type (i.e. extensions) are introduced upstream. I
updated the get-orig-source target in debian/rules accordingly.

* I'd like to push for a standalone mame-extra package. Nplayers is
still on hold at 0.147 and it's the major issue to a proper 0.148
release at present. I'd also like to include more stuff (mameinfo.dat
in the first place), and things would only get worse. I'm aware this
will pose some problems, so I understand we can't deal with before
0.149.

Man pages have been accepted/updated upstream in 0.147u4 so I removed
them as well. A small patch dealing with a new option introduced in
0.148 remains (already forwarded upstream).
