It happens from:
  Desktop -> Log Out <username> -> Switch User
  Applications -> System Tools -> New Login

I haven't tried any other ways.

No, I haven't found any keyboard input that works. I've tried
control-alt-F1 to control-alt-F8 and control-backspace. I don't remember
now if I tried control-alt-delete before giving up and pulling the plug.

I have an NVIDIA RIVA TNT 2 card and I'm using the nv driver.

In case it's of any help, the screen gets garbled exactly the same way
when I'm loggin out or switching of from GNOME, but then only for a
brief moment before GDM or the console appears.
