Best Practical recommend installing into a new directory structure and
migrating things over. There are some internal changes. Because of that
I've used request-tracker5 directories and allow for v4 and v5 to be
installed at the same time, like we already have for v3 and v4.

Database migration requires a set of patches to be applied.

Given the nature of the changes I suspect that a manual transition is
still the best approach.

Agreed. I haven't even looked in this area yet!
