adds an entry like

"* Rebuild for jessie-backports."

this is what is commonly used, not what you added

yes, in case there are changes you can edit it

I did pull-debian-source samba, and dget your version.
debdiff between them shows a deleted file.

somewhere in your workflow you deleted it.
