I've never been able to reproduce it, but it might be that I did not test
with kernel 2.6.25

It might be a kernel change which triggers a bug in shadow or bash, it
might also be a kernel bug.

I currently have no clue on what's happening.

I will install a 2.6.25 to try to reproduce it.

The PAM configuration is probably not relevant.
