OpenSER renamed to Kamailio

Hi,

You've probably seen my earlier commits renaming OpenSER to Kamailio.

The OpenSER project has to change its name, and picked up Kamailio as
a nice new name (see http://kamailio.org for explanations on the
meaning and origin of the name).

I'm starting to rename everything and prepare the transition. Don't
attempt to build working packages out of the kamailio/ tree, it won't
work for a while...
