Exim has behaved that way as long as I can think.

Please give proof that the log file still is open, which exim process
has it opened, and what the exim process is doing while keeping the
log file open.

I have never seen this issue on any of my systems.

This is not an acceptable fix as it disrupts exim operation and will
not help since I suspect that the log file is not kept open by the
smtp listener process. All other exim processes are "short lived".
